2018 Summer Camp Forms
Each camper is required to have forms on file before the start of his/her first camp session. (Note: Parent/child, Junior Curator, and Junior WILD Keepers classes do not require forms).
Forms are as follows:

- Camper information forms - fillable PDF forms. These can be filled out and e-mailed to email@example.com or dropped off at the museum.
- Camper Medical Form - This form must be signed by the parent and doctor. If you have another medical form that is dated within the last three years we can also accept that in place of this form.
- Allergy forms - only required if your camper has a life-threatening allergy that requires medication.
